  1. We are trying to help determine the validity of our DS-3025. This is question is specifically referring to the "Influenza" portion. My fiance (Brazil) had her medical exam in Brazil; we have the DS-3025 showing (with dates) she received DT, DTP, DTaP, OPV, MMR, Measles, Other MCV conjugate, and Varicella History is checked. For Influenza, however; in the blanket waiver column she has the letter A. At the bottom the US Vaccination Requirements box is checked. The doctor's signature and stamp are at the bottom.

     

    I am under the impression we do not need the I-693 form. But, regardless if we do or do not, is one needed to be submitted, even blank while also enclosing the DS-3025?

     

    Lastly.. we've been married about a week and a half and are now looking into the above as well as AOS at this time. If there's anything else pertaining to our situation it seems like we are forgetting, we'd so greatly appreciate your advice. Many thanks, VJ community.

  3. I am sorry just being curious about it right now. I'm traveling in 10 days with my sealed closed package. What is this copy of PSA birth certificate you are saying that I should have a copy?

    I'm from Brazil.

    I already did book for the first week of February, just a couple days from now, still I am so worried. Thank you so much for the reply. 

  5. I am traveling with my dog also In February from Brazil, as long as you the CZI corret and any other thing that the Airline ask you are fine I think. I even called the USA Embassy to make sure and because I am paranoid also, lol. Travel with United can be more difficult bc they ask Cargo if your animal is not a pet in cabin (my case) just dont forget ANYTHING and make sure your dog is older than 45 days  Rabies vaccine more than 30 days in the body of the animal and less than a year also.

    I used to work in this European airline so I know a little about it.

  6. Hi Im having the same kind of problem; I just found out now looking at my Visa k1 inside my passport.

    Our Visa K1 USA was approved I got the passaport with it inside and just notice that the last name of my Petittioner (my husband/boyfriend) is misspelled wrong, instead of Dannunzio Jr. they wrote Danuzzio.

    All the rest is correct. Do you guys think I should worried with this?

    Im traveling on February, the first week.

    I have a drama situation that I found out just now looking at my Visa k1 inside my passport.


    Our Visa K1 USA was approved I got the passaport with it inside ans just notice that the last name of my Petittioner (my husband/boyfriend) is misspelled wrong, instead of Dannunzio they wrote Danuzzio.


    All the rest is correct. Do you guys think I should worried with this?
