For Singapore, in paying the visa fee i understand that the K2 has to have a separate mrv slip. When paying the visa though, does it have to be separate? or consolidated in one receipt?

Hello. I'm on a K1 visa and recently got married. I had a name change during marriage and currently my legal name is including my husbands last name on hyphen. I'm about to file I-485(Adjustment of Status/Green Card). The form asks for the current legal name which is technically my married name but i actually want to use my maiden name on the forms. Is this possible? Or do i have to use my legal name?
