Hi, FYI...When I paid my AOS fee, the cover sheet was available at the end of the payment process. There was an option to print or email the cover sheet.
I then used the AOS cover sheet for the AOS and IV packages along with the included document cover sheet.
From what I've researched, do NOT wait for the IV fee to send both packages.
