The price includes filling out forms I-693, physical exam, blood test (for syphilis / RPR) and also the skin TB test (TST/Mantoux).
The clinic was willing to accept external lab results (I wanted to have iron-clad proof of immunity for Varicella (chickenpox) instead of just relying on "verbal history") and for QuantiFERON TB Gold - instead of TST, to which we would test positive (coming from a BCG-vaccinating country). We didn't want the extra radiation knowing that the skin test will end up being false positive, so searched on-line and with various labs/health departments.
There are private labs who offer the QuantiFERON tuberculosis test for about $150 (after coupons), but we also checked with local / county / adjacent cities' health departments who run disease / TB prevention programs and found one who accepted even non-county residents for $60 for the QuantiFERON test. We ended up getting to one of the Theranos locations for $42.18 per person, and also added a Varicella test for $8.77 to eliminate any subjectivity. Got the results in 3 days via Theranos' app, brought them back to Prestige Primary Care - who were happy to accept and attach them to the form.
Overall, highly recommend the clinic. After much research (believe me, tens of hours on the net and on the phone, registered as a new user just to share the information with the community which helped me learn) this was hands down the most cost-effective solution I could find in the whole Dallas - Fort Worth area - on top of excellent, professional service. Hope this helps others, and Dr. Aziz will get the volume of clients to keep the costs down. We were quoted up to and above $1,000 for the family, before any tests or vaccinations (!) - so I voted with my money for Prestige.
Last thing - check with your employer whether they will cover the costs (check-ups and vaccinations for you and the family) - I tried my luck and was pleasantly surprised!
