I've been asked for the same thing by Abu dhabi consulate for my K1 visa application for my fiancé. I think, this form is no longer needed and it has been a mistake their end by sending us an un-updated document list. Below is a blog which confirms that only DS-160 is required for K1. However, I would appreciate if someone more expert chimes in.

One thing that I have a hard time figuring out is if the k-2 visa interview has to take place with the K-1 visa interview. Could K-2 visa interview and all the related details happen sometimes after k-1 visa is been issued and we have got married?
