Hello digest,
We left that blank and only completed the part that has to do with the vaccine waiver. Like simoncini03 earlier post - we included supporting documents for why we applied for the waiver.

Thank you simoncini03 , I did not think so either however following the guidelines on VJ I thought I should ask and verify.. We will be sending the package with the checks today. Thanking you once again.

My vaccination waiver got approved and I offer help to anyone who has trouble preparing the waiver package.
I have posted on here as army16 before.
