So basically my vaccination report was incomplete because the result section was not ticked wether it was complete or incomplete. All the vaccine boxes required are checked, just the flu vaccine was not because i had my med exam april. Also the waiver thing where they put letters where not filled out. What should the civil surgeon fill out in the fir i693? The vaccine part only? Ill just tell him to transcribed my vaccine report on the form i693?

-i had the Tdap, MMR, and Varicella hx checked

-the name, sign and date of the examining doctor complete

-the summary for immigrant visa applicant is not checked (whether it is complete or incomplete)

I am a K1 visa holder and already applied for AOS last july. My medical was completed in the philippines last april 8,2015. I had my biometrics done. I received a letter from uscis that i didn't submit for i693. Ive read before applying for AOS that i am not obliged to submit i693 since i am a K1 and applied for AOS with my medical less than a year old. Did they sent me this letter because the SUMMART FOR IMMIGRANT VISA APPLICATION BOX was not checked if it is complete or incomplete?what could be the best thing to do?thanks in advance for those who would enlighten me with my situation

In the notice that they sent me , it says that if i receive an interview notice in the mail, i must bring my updated form i693 to the interview; or if my case doesn't require an interview, i will receive an RFE for form I693 with instructions where to send it. That i think really requires me to have that form I693, my question now is that what would i tell the civil surgeon to do?or should i call the NCSC for my case to clarify which thing will i be letting the civil surgeon to sign in my I693?thank you kaydeecee

So what should i do then? I will go to a civil surgeon to let my I693 signed? They are asking me in the later part to submit a completed and signed I693,so should i go to a civil surgeon to let it be done so as to comply with their request? What should i asked then to the civil surgeon to do? Just to transcribed my vaccination card to the form I 693 and then their sign?

My question is does that mean i need to do the whole medical exam again just to complete that I693 form?i completed my medical before in the Phil.

hello! Me and my US citizen husband just got married about a month ago, we already filled my AOS. My question is , is it ok that th name in my passport copy that i submitted is my name when im still single? And 1 thing more , my passport is still valid until 2019? Can i still use it even if i am already married or should i change my name already to my married name?

i just received my social security number. my question is they mistakenly put a wrong middle initial in my card,i said to them at stated them clearly what should be written in the card but got the wrong middle initial written, can i apply already my I-485 because i already have the ss number even with wrong middle initial?Or should i coorect my initial before applying and wait again for that to be changed?

what will i do in the case that i didn't file for social security after 2wks in arriving in the US?it so happened because we arraged first my marriage with my fiance and after that we plan to do it. Hi, i am a K1 visa holder and is already married to my fiance, now we are starting to apply for my change of status. Upon my application before of K1 visa my husband got his uncle as his joint sponsor for form I-134 affidavit of support. In applying for change of status he is still getting a joint sponsor, is it possible to get a new joint sponsor for I-864 affidavit of support for his uncle is not available to do it so?or should it be the same as of the K1 visa form I-134 affidavit joint sponsorship? Hope you could help me with this one, thank you
