    Steps in CFO Guidance and Counseling Program

    Step 1: Guidance and Counseling at SMEF-COW

    Accomplishment of CFO Guidance and Counseling form

    Payment of P250.00 counseling fee

    Attendance in the counseling: group or one-on-one counseling, including IDC

    Issuance of SMEF-COW Certificate of Attendance

    For Clients who will apply for passport and visa:

    Step 4: Registration at CFO

    - Payment of P250.00 registration fee

    Counseling Requirements for Filipino Spouses and Other Partners of Foreign Nationals

    Two (2) valid identification cards with photograph (i.e. passport, driver’s license, NBI Clearance, SSS or GSIS ID, company ID, professional license card);

    Duly completed Guidance and Counseling form (available at CFO or SMEF-COW);

    If married, certified true copy and photocopy of Marriage Contract on Security Paper from the National Statistics Office (NSO), or the Local Civil Registry Offices (LCRO); or original and photocopy of Report of Marriage duly authenticated by the Philippine Embassy/consulate (if married abroad);

    Other documents (i.e. divorce or separation papers, photographs, parental consent, letters, other proofs of identity, civil status, etc.) as may be required by the counselors; and

    Payment of P250.00 counseling fee.

    After receipt of the passport and visa, the Filipino spouses and other partners of foreign nationals will have to register with CFO prior to their departure. Registration is done either in CFO Manila or Cebu Extension Office, Monday to Friday, 8:00 a.m. to 5:00 p.m. An emigrant registration sticker is affixed to the emigrants passport as proof of registration.

    Registration Requirements for Filipino Spouses and Other Partners of Foreign Nationals

    Original Passport;

    Original and photocopy of visa;

    Original and photocopy of Immigrant Data Summary for US bound/Confirmation of Permanent Residence for Canada bound;

    Original CFO guidance and counseling certificate;

    Duly completed emigrant registration form; and

    Payment of the authorized registration fee.

    * CFO has a Satellite Office within the C.O.W. building

    For the immediate issuance of CFO guidance and counseling certificate and registration sticker after completion of the guidance and counseling session, CFO has established a Satellite Office within the building of C.O.W.

    My brother used this method, the local NSO employee say it usually take less than a mo.

    2. I recently applied for my birth certificate and upon receipt of my document, I found out that the first name is blank. How can this be corrected?

    In order to supply the missing entry in your birth record, you have to file an application for a Supplemental Report at the Local Civil Registry Office (LCRO), where your birth was registered. You may have to bring supporting documents, e.g., baptismal certificate, school records, etc., as these may be required in filing the application for supplemental report. The LCRO will then be submitting a copy of the Supplemental Report to NSO, along with the civil registry documents they regularly transmit to the agency.

    This procedure also applies for birth records with "Baby Boy" or "Baby Girl" written as the first name for births that occurred until 1992. For the years thereafter, 1993 to the current year, correction must be done through RA 9048 (Clerical Error Law).

  3. Can anyone tell me about the seminar my fiancee has to attend before leaving to come to USA? Do i need to attend also? They say no they just need a copy of my passport. Do i myself need any documents? Any one who has already attended this seminar in manila recently could give me any info I would greatly appreciate it. She has her visa now i'm just going back and we are coming back together to start our lives together and i can't wait :-)

    No, you do not need to attend the CFO Seminar. It is for filipinos going out of the country to live or work. Your fiancee will need her passport and visa she received from the embassy after her interview. The fee, I believe is p400 for the counseling and p400 for the registration.

    Your fiancee will fill up form in the CFO. She needs to know some basic info about you before she go there such as your middle name, your job, birthday etc...

    dont forget to bring the passport, i think they also need 2x2 photos (not so sure), and some important documents, and provide some photocopies of everything, coz when we went ther,e the photocopy machine shop are so far from that place so its quite hard.

  5. No. you dont need to bring the birth certificates of your children from previous marriage unless you have plan to get them someday. If u have children that u really own both of you, i mean u & your wife together, u can bring that. For instance, my husband have 18 yrs.old kids living here in the US but we didnt bring her birth certificate while me I have a 6 years old son (that im fixin to petition) I bring his birth certificate in our interview.

  6. I'm really concern about the Indonesian who had been hit by the Tsunami. According to the news its almost 300+ the death toll and hundreds are missing. I hope this will not happen again. I send my symphaty to all people who live in Indonesia. Pray is the only thing I can help right now.
