    My fiancee arrived a couple of days ago and yesterday we went to get our marriage license. A big surprise!! They asked if we wanted to get marry right away and we said yes, so we got married :)

    We want my wife to change her last name so we are not sure how to go about this so It won't interfere with the next step which is requesting adjustment of status.

    Do we need to change the name on our marriage certificate? Do we change the name at the social security office? How do we file the adjustment status under the new name?

    My fiancee lost all records of previous vaccinations and I want to know what vaccinations are required? I know she does not need them for the interview but I am wondering if it would be better to take them in Ukraine before she comes.

    Can someone give me a list of the basic vaccinations required? Can she have them done at the medical examination place before the interview?

    irina_frank do we need to pay $265 for my fiancee interview and another $265 for her child? or only one fee for both?

    Another question, would you happen to know how to get the police certificate? She lives in Kremenchuk (Poltava Region) and I am not sure if she can get it there or she has to go to Kiev.

    In Part 3 (Other Information) Question 4 says:

    Indicate which waiver applies:

    - Multiple Flier. No Disqualifying Convictions (General Waiver)

    - Multiple Filer. Prior Criminal Conviction for Specified offenses

    - Multiple Filer, Prior Criminal Convictions Resulting from Domestic Violence

    - Not Applicable, beneficiary is my spouse

    In my case the multiple filer answer does not apply so my best option is Not Applicable, beneficiary is my spouse. What throws me off is the last part "beneficiary is my spouse" we are not married yet and I am not sure if this question should just be left blank or if I should choose this answer..

    Can someone please tell me what's the correct way of answering this? I read that most people just choose the last option but I am not sure if this applies to me.

    I filed for a fiancee visa back in 2001 with another person and when she came to the US, we decided not to get marry (I did not report back anything because she went back to her country).

    Now, I would like to file another fiancee visa with my new fiancee. The form ask if you had previously filed, you need to disclose the information.

    My problem is that I did not save any records of my previous filing and I don't know the date of filing. Should I leave the date of filing blank? Would this complicate my current fling?

    I am tempted not to disclose it to avoid possible problems but at the same time if they find out I previously filed for another person I don't want my application to be denied.

    Any thoughts?

    I am getting ready to fill out my I-129f and I have a few questions regarding my fiancee's child:

    - Do we need to submit pictures of the child with the i-129f application?

    - Do we need to translate to English his birth certificate?

    - On the form, I see that there is a place to put his information but do we need to submit any other forms or documents for him?

    Your help is greatly appreciated.
