I am filling out an i129F for my Fiance.  I brought my ex-wife here on a K1 years ago but I don't have her A number for the i129F and she wont give it to me.  Any suggestions on how I should proceed?

When filling out US Government forms they ask for the person's middle name. Usually the patrynomic name is thought of as the middle name.  For example in Olga Ivanovna Petrova  the derivative of the fathers name Ivan is the patrynomic name. However, This name does not appear on some Passports.  If the foreign fiance has this name on some Ukrainian documents but does not have this name on their passport should we be using it to fill out forms?

My new wife wants to use my last name as her own on our I-130 and 325a. We have not been able to register her name change in either of our countries and her passport will still have her maiden name when she arrives back in the U.S.

Is it ok to list her current family name as mine and how should she sign her 325a?
