I just got finished going to the bank to get this letter. I'm not sure if you're trying to do this in person or over the phone, but I would recommend going to your local branch and meeting directly with a bank official. Do not attempt to get this information from a teller, but an actual banker, manager, etc. I went in today, met with a banker and they wrote the letter right in front of me. I belong to a credit union.
I am convinced this is a mandatory document as it is specifically listed on the Form I-134 instructions along with the letter from your employer and W2.
