Top right of the form they are asking A#. Is that from Citizenship Certificate?

As long as your wife is not in the states , then leave number 22 blank

Concerning form G325A that's the file number , I think you get it when you file the form , they give you the file number to check on line. Actually we hired a lawyer to do that for us , I am the spouse and I am not in US
