I've done my ROM last year.

I sent my requirements an hour after they posted the schedule in Nashville TN Outreach. 

And on the renewal form I used my new last name not coincided to my passport.

I forgot to ask this to my first message...

Should I send a copy of my expired green card?

Hello, I just wanna ask about Renewal of Passport in Philippine Embassy Outreach Program.

Last month I emailed my requirements to get an appointment to their Outreach Program

but unfortunately I didn't receive any confirmation for appointment and I'm not included on their final list.

I want to renew my passport (almost expired) and change my last name at the same time to my married last name.

My current passport last name when I was still single.

Documents I sent:

1. Renewal Passport Form ( I used my married last name here)

---After I failed to get an appointment I have doubts if I filled up the form correctly...

Question 1: Do I need to fill up the form base on my passport last name or my married last name?

2. Copy of Passport Biographic Page

3. Birth Certificate

4. Report of Marriage

5. Notice of Action (letter of extension for my green card), I didn't include my green card copy since it is already expired but ROC already filed.

Question 2: Do I need to send/mail the passport fee?

Hello, I just wanna ask about what name gonna use when you buy a ticket. I have planned to vacation in the Philippines this summer.

My green card name is my married last name but my passport  is still on my single last name

And what other documents do I need to present when I come back in the US.

The instructions said to "Embassy of the Philippines OR " Philippine Consulate General"

I'm from Georgia and I'm going to send my report of marriage to Washington DC where we are belong in this Embassy Consular Jurisdiction.

I'm here again, I'm on preparing to report our marriage but we are delayed on reporting it. I read some posts that if delayed you need to have a notarized affidavit.

My questions are:

1. How many original and photocopies for the Notarized Affidavit of Delayed Registration of Marriage?

2. Where you can go to get the Notarization and like to have an idea if how much it costs.

RFE dated December 2, 2015

and green card - resident start -January 4

RFE dated December 1st week, 2015

My status online - January 2nd week, 2016 Card was delivered to me by the post office.

I am here again and wanted to ask about my RFE for my green card (Adjustment of Status).

I submitted all the documents needed for my AOS.

But unfortunately I received an RFE last December 2015. (They requested more evidences to prove that marriage is genuine, like deeds, insurances, photos,

etc...

I am still processing and completing the documents they requested and NOT YET sending those documents

BUT last week (2nd week of January 2016) I received my green card. I was so much happy

And just wonder if I still need to send those documents they needed.

I want to inquire to the immigration office on what to do but don't know how to contact them.

I only have SSS card but the bank needed two. Are those EAD/AP can I use them as a valid ID if I get them?

I just wanna ask if the consule required to have the joint bank account for AOS (green card) interview?

I already done my biometric the other day and now preparing proofs for my coming interview. We went this morning to the bank to have joint account but the bank needed 2 ID's But I didn't get the State ID or driver's permit because of my I-94 already expired.

I just like to ask some ideas. About what are the requirements in joining account.

I read some posts here that it is needed for AOS interview. And read also that one of the bank required is ID card. Tried to get the State ID but need valid I-94 and mine is expired already.
