    Medical Complete. Asked for a copy of the DS-3025, but was not given one even after promised by the front desk! They said since we did not need vaccinatio9ns none was filled out. We showed them the proof of vaccinations, bu said since they did not administer any, they can't provide a form! Explained it will be needed for AOS, but still was not given any! @$!#*!!
