Yes, they sent the entire application package back along with the rejection letter. It looked like it was already scanned in (all pages had a barcode/info on the bottom). I resubmitted the same package and they accepted that. The only form that was missing was the credit card authorization form, which the rejection letter states was destroyed so I sent in a new one.
