You don;t absolutely need a I-134- with the DV lottery visa, other options can be considered such as a job offer, or assets (ie savings you can use to live on for a while).
But, US Embassy in Tbilisi, Georgia asking affidavit support-that's one of official requirement for citizenships of Georgia and Azerbaijan who admitted for interview there.

Concern the Police Certificate. Right now very easy to get that certificate. Just have to go to the new government service -web site - asan DOT gov DOT az - located on
address: Baku, Az 1078, Akademik Hasan Aliyev street, 36 (former Inglab str). But, you need take you ID card, filling form there and the next day you can get it. No charge.
