I'm also having trouble finding a civil surgeon. I called around today, they said they offer medicals and vaccinations, but not just the vaccinations. They told me to go to the county health department, who didn't answer. Does the health department have a civil surgeon to sign the I-693? It seems like from what I read you go pay for the shots at the health department, and then you pay a civil surgeon to sign the I-693, which I find rather ridiculous to pay the civil surgeon to fill out the form. That is if I can even find one willingly to do that and only that without medical and not charge an $200+. Looks like vaccinations will cost $285 at county health department already. And to clarify, I've read that you don't need the second doses for vaccines like MMR. Just the first is sufficient, is this true? I also have no ds 3025 like everyone else seems to.

I got an apartment on the day of my fiance's POE, but I couldn't add him to the lease because he didn't have a social security number for a background check. Then I was told that I wouldn't be able to add him to the lease until he physical has a green card. However, on the application it says I just need one of the following documents:
1. Form I-551, Alien Registration Receipt Card
2. Form I-94, Arrival-Departure Record, with the following annotations:
(a) "admitted as refugee persuant to section 207"
(b) "section 208" or "asylum"
© "section 243(h)" or "deportation stayed by Attorney General"
(d) "paroled pursuant to sec. 212(d)(5) of the INA."
3. If Form I-94, Arrival-Departure Record, is not annotated, it must be accompanied by one of the following documents:
(a) a final court decision granting asylum (but only if no appeal)
(b) a letter from a DHS asylum officer granting asylum (if application was filed on or after October 1, 1990) or from a DHS district director granting asylum (If application was filed before October 1, 1990)
© a court decision granting withholding or deportation; or
(d) a letter from a DHS asylum officer granting withholding or deportation (if application was filed on or after October 1, 1990)
4. Form I-168, Temporary Resident Card, which must be annotated " Section 245A" or "Section 210"
5. Form I-688B, Employment Authorization Card, which must be annotated "Provision of Law 274a.12(11) or "Provision of Law 274a.12"
6. A receipt issued by the DHS indicating that an application for issuance of a replacement document in one of the above-listed categories has been made and the applicant's entitlement to the document has been verified.
7. Form I-151 Alien Registration Receipt Card.
Will any of these documents be issued prior to the AOS interview? That way I could bring our lease as evidence.
The lease says having anyone not on the lease move in would result in eviction so I don't want mail for my fiance to come to our address. So for the AOS I have my parents address both for place of residence and mailing address (to be consistent since not all of the forms have a place for both). Will that be a problem? I've heard that sometimes Immigration will come to your door to check if you live there. Is that to remove the conditions or before?Thank you

i believe that invitation letter is what they want you to take to the interview. they consider it "packet 3"

while you do need other stuff for interview obviously, i don't think you need other letters from immigration, like the nvc letter stating the numbers. you are right about noa2, proof of petition approval

That's what i get too. I don't think the size matters. It's official and includes all relevant information. I'm sure ticket stubs are just as good as the boarding pass, since no where does it actually require the boarding pass.

Okay, then I will put down my 2012 earnings and attach the tax transcript to my I-134.
My sister will fill out an I-134 and just get an employment letter.
I might also request a letter from my bank.
Is that sufficient evidence? I'll probably have my sister attach her tax transcript too just in case. Should I fill out an I-864 too, or is that overkill?

I work at a salon and file my taxes as independent contractor which counts as self employed. I started working May 2012, so my 2012 tax return doesn't show above poverty line. My 2013 tax return would be above the poverty guidelines, but I don't have the tax return for that yet. I receive a check biweekly (although the amount varies).
How much should I put down for my annual income? Should I put 2012's or 2013's estimate (which I would take from adding jan-august 2013 + september-december 2012)? There's no way to prove 2013's income unless I attach all the checks and explain how i got the estimate, but it's not very official...but I guess I could include a letter. I could ask the salon owner to write a letter, but none of it is in the approved evidence.
I will definitely have a cosponsor, but I'm still not sure what to put for income for my I-134. Please advise, thanks.

I have few questions about filing out the I-134 form.
First, I want to clarify that the I-134 is required for the Mexican consulate for K1 visa and not the I-864.
Second, on question 7, it says "I am employed or engaged in the business of ____ with ____." I'm a nail tech at a salon, I file my taxes as an independent contractor so technically I'm self employed. What do I put in these blanks?
Also for 2012 I only worked part of the year, so my this year I should make more and set me above the poverty guidelines. I receive a check every two weeks that can show my projected income for 2013, but is this acceptable?
Are bank statements required if they are not necessary to qualify above poverty guidelines. I will have my sister cosponsor but does she have to get a letter from her bank? Can she put $0 on that line about her bank even if she does have some savings in the bank? Since her income alone should be sufficient.
"I have personal property, the reasonable value..." I don't have anything significant, should I leave it blank or put zero?

I know during naturalization, there's an option to change your name, which is a nice option for those who want to change to an American first name. But wouldn't it be simpler to change it from the beginning? That way my fiance wouldn't have to change his name on all his documents (like SSN, bank accounts, marriage certificate, passport, driver's license) later.
I've read that he might be able to during the k1 visa interview or the POE but can't find much details on the process.
I haven't filed my k1 petition yet. I wanted to see how would be the simplest way to change his first name. I think a court order name change in his country before filing the k1 seems a bit of hassle (petition, hearing, publication, etc), so I'd like to avoid that. If there's an option to change it some time during the K1 process, I'd really appreciate if someone could point me to how. Thanks.

We got a letter about a month ago saying the extension of residency will now be 18 months instead of the original one year. Fortunately, we already have been approved for naturalization.
