    Packet 4 is just an email saying that you have to wait 5 business day and then call the number they gave you. I sent my fax on Dec.18 and received this email on Jan 10. The important thing is that you have to wait to get this email because if you call before you received the email, they cannot find your data in their system and cannot schedule you an appointment.
