Is she mainland or Okinawa? I ask because we just had it translated. I could give you a place on Okinawa, but Mainland I dont know. I have not used the form from the embassy. I would say , due to them wanting so many copies of everything that, That your best bet for having them accept your paperwork the first time is to have professional translation as we did. No requests for more info that way....
Long winded but hope this helps.

I hope this helps... I just recieved a letter from the IRS on a correction to my tax return for 2012. I had originally filed Married filing joint, but my wife Is not here from Japan yet. If your spouse has not yet recieved a ss# or taxpayer ID # then she does not exsist as the IRS sees it. You will not be permitted an exemption for her, and you will be filing as single. So if she has an income it is irrelevant to the IRS unless she or he has a ss# OR TAXPAYER ID.
My tax man didnt even know this, as I claimed her on my taxes for 2012 and declared her income, to no avail.
Hope my trials with the IRS helps someone.....

Narrow minded statements of a past president, not pertinent to the world in which we live today....

Wow,... I believe, but as no persons info on here is written in stone, that Guam is a us territory and uses dollars for currency and Im reasonanly sure no passport is needed for travel to this island from a US port. If a brief stay on Guam qualifies for a re-entry into the US, then I presume u would be fine. However please refer to the first statement in the post.

We got married in Oct, in Okinawa, we opted to change everything immediatly after our ceremony. Passport only took two weeks to change to new last name. We have used her new last name throughout the entire process, and all has been 1, 2, 3, right along. I guess I would stay as it is now, as the other person prior said. Dont confuse the situation middle way through. But you could always call the NVC and ask. Good Luck!

Police in okinawa told my wife they need some form from the NVC to get her a police crime certificate, Yomitan village to be specific.... I told her to go in and get them to give them to here any way she could....I am not aware of any form that she would need to give them to "authorize" them to do a cert.
Anyone know or heard of this #######?

No problem,......Make sure on your DS-230, In the area that you list places lived since the age of 16, The start date should be the birthday of the individuals 16th birthday forward. Not from birth as you would think.
I know this now cause we got ours returned to us to correct due to this miniscule error.
Good luck to you....
