hi! I just want to ask about the DS-11(Application for a US Passport) there's a new instruction that starting February the form must be completed electronically and printed. I will apply a US passport for my son.. i dont want to do it online.. I just want to ask how to download it so that i can type it electronically and have it printed. please help me on this. thank you so much!

I also want to ask on the item no.6(email address), no. 7(Primary Contact Phone No.)and no.8(Mailing Address) of the Application for a US Passport of my son..... whose email address, phone no. and mailing address will appear since my son is still 2 months old. do i have to leave this blank or do i have to fill it up and put my email ad, telephone no. and my mailing address? please help me. thanks

hi! i will be applying for my son's US Passport. Im just confused on the no. 5 item about Social Security Number... do i have to just put zeroes? as what ive read from the Instruction that states" Failure to provide information requested on this Form including your Social Security Number may result in significant processing delays and/or a denial of your application.
