It sounds like the koseki (戸籍), then. If in Japan, you can get it from the local city hall where she is registered, I believe. If abroad, I don't know: I guess you should call the local consulate or embassy and ask them.

Thanks for your help. We are now in process of trying to get a certified copy of the koseki. 4 more documents, and completing interview 2 and we should be hopefully granted a visa for my wife. What a lengthy, costly, and cumbersome process. Can't wait til it's over. Thanks again!

It sounds like they are asking for the the koseki (戸籍), but possibly the juuminhyou (住民票). What are they asking it for?

This document, the household registry, is required to support a death certificate of my wife former husband. Thanks for replying to my post.

My wife is being requested for an "original or certified Japan Household Registry." What is ths document exactly and who does she need to contact in Japan to get this document? Thanks in advance for any help which you may be able to provide. -Chris
