    I thought I would give a little update... We had our appointment with the civil surgeon today - Elizabeth Neel, Doctors Express Urgent Care in Cary (101 Maynard Crossing Court, Cary, NC 27513; Tel (919) 651-9867), everyone at the surgery was so nice and helpful!!! Most importantly - they were very knowledgeable about the requirements for Form I-693. I gave them my passport and combo card (for work authorisation/advanced parole) as ID. The combo card was needed as it provided evidence of my married name. I forget to take my Social Security No, which they did ask for, but they processed me without it (thankfully!). I was asked to complete the usual registration forms for a new doctor, as well as Form I-693. I had taken along my own copy of Form I-693 with my details typed into Part 1, which they used. On completion of the required paperwork, the lady at the desk asked me for any evidences related to my vaccination history - I provided the vaccination record I received from the civil surgeon in London, a letter from my GP in the UK with a summary of my vaccinations from birth (I obtained this for the purpose of my K1 medical in London) - as well as a note from the surgery in London confirming the vaccinations I had during my medical - my second MMR and Td/Tdap. My husband and I sat in the reception area as they processed all of my information information, both of us expected to get called in to see the doctor once everything had been processed, but we were called to the desk and asked to pay a consultation fee of $65 - and were then handed our completed and sealed letter for USCIS!!!! It turned out that the lady at the desk we had been dealing with was our civil surgeon!! I had expected a more onerous process and asked her if everything was complete - she smiled and told us that everything was complete and that we had more than enough evidence!!! We both praised God for his goodness in it being so straightforward. I was concerned that I would need more vaccinations...and that as they had quoted over the phone it could cost up to $300...
    I now aim to send the sealed letter as signed for recorded delivery, and will include a cover letter and a copy of the Intent to Deny letter... Thanks again everyone for your kind assistance!! I thought the above summary would be useful, as it transpires that a few more North Carolina filers on here have received the same Intent to Deny letter... All the best to everyone!!!!
