It allows someone to make an inquiry on your behalf. I think our senator had a copy on his website or if you don't find one on yours they should be able to send you one. It needs to be signed so that is why we faxed a copy of it in. Good luck, I it works for you too.

Here is what happened: My fiance emailed and faxed a letter along with the privacy form last Thursday. A lady from the senators office rang my fiance on Friday to say that she would call on Monday and ask what was going on. The Monday she emailed saying that she had insisted they process our case and that she have a response by the end of the week (at this stage we were uncertain if that meant a final decision or if our case would just be assigned to a case officer). And amazingly she rang on...

I think we just struck gold with the woman at the senators office, she did an amazing job insisting that they process our case, and less than a week after contacting them our case was approved. We feel very fortunate and grateful.
