Notarized if your parent(s) do not accompany you to the CFO, with two witnesses. I have heard of at least one case where the CFO would not accept the written affidavit and required the parent to appear in person. Perhaps the affidavit was not executed properly, or the CFO had some other reason to reject it.

Here is a sample of parental consent, I don't know if it needs to be notarized by a lawyer

but I prefer to have it notarize, won't cost too much if you prepare your own letter.

I am 21 and I have heard that I need to bring a parent's consent for the CFO seminar. The problem is I don't know where I can find form or whatever is it needed to get this Parental consent. Could anybody be kind enough to show me their format or sample of parent's consent.
