This is just a ploy to get more money. I paid 300 for the right to throw the party. If you are really worried email the embassy yourself and they will get back to you within 24 hours. Just have all the forms on packet 4 and you'll be fine. The US Embassy knows about the corruption there and will tell you that the form is not needed.

I'm in the same boat as you. Were having our engagement ceremony on March 31st. I talked with the embassy tonight and they stated the same thing that you received in your email. I plan on just pdfing the case number I received from the Texas center. I hope that will be enough.
