  1. Hello, I am about to file Removing Conditions that will happen this month, however, I have a question regarding form I-751. My question is,

    Except follow my husband's family name, and my maiden name, I also have an American English name called Bonnie that been with me for more than 10 something years. During my K-1 or SS application, I did not mention it as it was NOT put on any of my legal documents, like ID card or passport, etc..

    But the name been used and called by my friends and husband's family, and they wrote cards or gifts used the name too, plus it's also use on and called by my co-workers here in the States.

    Should I put it on form I-751, 3.a. and 3.b.? as most of my bona fide relationship evidences show's the name I am also worried about will they want me to prove the name is me, the one thing I can think about to prove is my company's offer letter when they hired me. Will that be enough?

    I would be very appropriate if you could help me.

  2. 1. You can try to contact your medical doctor in HK to ask if he would be able to re-issue a copy of DS-3025, he/she would have your file in their record. If he would be able to do so, then you don't need I-693 anymore.

    2. If you have to do I-693, you ONLY fill part 1, a Civil Surgeon will fill ALL the other parts of I-693. You need to contact this CS and tell them to correct the missing parts, you PAID so it is his/her duty to complete it. If he refuses, ask for a refund and find another CS.

    3. You don't need it and CS will mark "WAIVER" on the vaccination list on I-693.

  3. Hi everyone, I have receive lots of useful information for my green card application(K1) here. Thank you so much!!!

    Now I will need your help, please give me some advice.

    I have received a reference today for my I-485 application, it just so confused me!! below is part of the infomation,

    Request for a additional evidence(I-485)

    This office is unable to complete the processing of your application to Register Permanent Residence or Adjust Status, (Form I-485) without additional information. Please submit the information requested below.

    A review of the record indicates that you entered the United States pursuant to a valid K-1 nonimmigrant visa. However, the authorities where you were admitted or the U.S. Department of the States did not forward the medical form DS-3025, Vaccination Documentation worksheet that was completed perior to visa issuance in conjunction with your K-1 entry. Although you submitted Form I-693, however, it was not properly certified by the surgeon who completed the form. As such, the following age-appropriate vaccination(s)are required for the applicant:

    [*]DT,DTP, DTap,Td or Tdap

    [*]MMR

    [*]Varicella

    [*]HPV

    My questions are,

    1. I did not receive the medical form DS-3025, never seen it! I did research on here, lots of information showing that if I did not have the DS-3025, I will need the civil surgeon to filled up the Form I-693 Part.1 Information about you (signed in front of the civil surgeon doctor) and the vaccination record ( I already had body check in Hong Kong, and it less than a year).

    2. I did went to the civil surgeon here to filled the Form I-693, Part.1 Information about you and the vaccination record. when I looked at the copy of the I-693 the civil surgeon gave me, for Part 3. Civil Surgeon's Certification was blank, the doctor did not sign and fill up the information. was the part.3 suppose be filled up by the doctor even I only asked to do the transfer vaccination information from my owe Hong Kong record and one shot I did in here area health department just to save money!!

    3. regarding the HPV shot, I read it here on VS.com. information shown I DO NOT need it!! is that right? why they still mention it??

    if anyone have similar experience please share, thank you and much appropriate it!

  4. I'm having from mother fill out the I-864 to sponsor my spouse. My mother jointly filed for taxes with my step father, but her individual income meets the requirements. So, if I list my step father in line 24 part b do I need to have him fill out an I-864A??? I guess I feel like I have to put his name there since they jointly filed for taxes. So do I have to check line 24 boxes d or e, or can I just leave these boxes blank? Thanks in advance!!!

  5. Hi everyone, please help if anyone have any experience with civil surgeons in New Orleans, Louisiana which in a reasonable price? ( K-1 visa holder , currently need CS for my Vaccination transcription or check)

    tried to call health department and unit close by my house to get the blood test and shots, but they said they don't provide service or service only for age under 18. please help. many thanks.
