    Fiance programmed his medical for this Thursday,but he will postpone it for next Monday...time is going fast :)

      My fiance went to get his xrays today and bloodwork only because he didn't have time for the rest, but he'll see the doctor on Monday. Good luck to your fiancee :) I didn't find the forms. Only D156, but I get errors. I emailed the embassy. So they emailed the D156, D157 and the supplement form, too. I think that's all the forms we need. I only asked them for those.
