  1. That is a great thread!!!

We are looking for a reasonable CS in the Seattle area. If any are known, the info would be much appreciated. We will post back on the thread above with the results of our search.

    TIA.

Hi.. I recommend Dr. Efren Caratao, i went there last March and i only pay $35. The clinic is located in kent, here's the number, 206-852-2770. Just tell the receptionist that u just want to transfer ur vaccine record to I-693.. Good luck! :)

  2. Hi guys,

This is going to be my first job interview here in US. I don't know what to expect. Please share guys so i can get some pointers..

    Thank you much! :)

  3. I had my biometrics as date stated on my appointment letter at Tukwila, Seattle. It was long line but able to finish it. I arrived half an hour before the time given. It went well :-) Goodluck!

  4. Hi guys,

I went to SS office a week ago to change my last name and status.. I have noticed on the letter they gave that my middle name was not change to my last name when i was still single.

    For example:

Maiden name is Lorien Ocampo Dizon, since i already married it should be: Lorien Dizon Bennett..

but what reflected on my SS is Lorien Ocampo Bennet.. Since i came from Philippines and the practice their when u get married is that you maiden surname will now be you middle name.

Then last friday i tried to get a state ID, but when i submitted the form, which i write on the papaer as Lorien Dizon Bennet, the officer told me that it did not match on the SSN that i gave, maybe because of the middle name.

    Now my question are:

    1. Should i go to SS to change my name again, this is the third time i will be going there.. Does middle name is important here in the US?

    2. If i stick on the name on my SS and put it on my Immigration form, will there be a problem? will the middle will be an issue with them?

    3. Just in case my EAD, AP and green card be issue under the same name on my SS, will i encounter a problem when i travel outside the US? coz my passport is in my maiden name.

Hope you guys can help coz im planning to send my AOS documents my next week. Thank you much and have a great day to all.. :)

  5. I'm having trouble finding the answer and I see conflicting info, including conflicting with what's on ssa.gov. My fiancee gets on the plane in 2 days to fly here. I plan to get her the SSN right away (working with it is not important at this time). I am planning on us bringing these to the office:

    - Her passport with K1 visa in it.

    - The I-94 form.

    - Her birth certificate.

    - A filled out SS-5 form.

Is this enough? - requirements above is enough..

Are they going to take the birth certificate? She only has 1 certified copy of it. - i don't think they'll ask for it.. anyway just bring it to be safe.. And in my experience, the SS personnel will photocopy all you original documents then they'll return it to you..

How long does it normally take? We are moving shortly after her arrival and don't want anything mailed to the old address accidentally. - it takes 2-4 weeks before it arrive.

Also, after getting married, we have to return to get her name changed on it? What is needed for that, a certified marriage certificate? (And do they take this?) - what i did yesterday, was i return to their office presented my certified marriage license, again they will photocopy it and return the original to you. They also asked for my passport & I-94 form.

Speaking of moving, how do we change the address with any government office(s) that need to know about the new address of a newly arrived K1 visa holder? - We also move after the marriage, since i came to SS office yesterday i told them that i change address, so they just asked for the new address then update on their system.. But someone told me, you can do the change of address online.

    Thanks.

By the way i read on the forum that you should wait atleast 2 weeks before applying for SS#, so the info of your fiancee will be surely on the system.

    Goodluck & Godbless..

  6. your fiance can try to schedule online. as long as you have your manila case number you can go for your medical. i never received my packet 3 from usem and im here in the US since december.

    goodluck!

me too, i never received such packet from them and im already here.. Philwithlove is right.. You do the online scheduling, if you success with the appointment then ur case is already in the system.. if not try again after 2-5 days.. for me its better to do the online scheduling, coz my experience with the call center is not good.. they know nothing about our case..!!

And with medical, you can start with it as long as you have the case #..

    Goodluck!

  7. My wife probably didn't need to submit I-693 eventhough her DS-3025 said incomplete (may be eligle for blanket waiver/not age appropriate). We ended up going to a Civil Surgeon in Lynnwood just to play it safe, and paid $50 to do the transfer. If you do see a civil surgeon, be sure to bring a copy of the DS-3025, and any other recent vaccination you may have recently had so they can transcibe it.

I never saw this link.. Thanks Reyan.. :)

  8. Well my decision is based off of most people whom have submitted there AOS package. They don't include the I-693 form and did not go to the civil surgeon. Yes I am taking a calculate gamble and praying not to get a RFE. Trust me I am not the gambling type.

Our DS-3025 states the same as yours. We are going to gamble. You do not have to follow our lead. If you feel safer getting the I-693 done. Do that.. We believe most medicals from SLMEC state the same as ours and many submit the DS-3025 only and are approved. Again this is our decision.

I Advise follow your own judgement and instincts because it is a choose. Wish us luck and I also wish you two luck and prayers. We are all on the visa journey and hope to have a smooth ride.

    God Bless

Thanks for sharing.. :) Goodluck to both of you too.. I hope we can finish everything smoothly.

    Godbless.!

  9. Hi plus0212

Please read this thread before going to the Civil Surgeon and paying there crazy prices.

    http://www.visajourney.com/forums/topic/101202-faqs-forms-i-693-and-ds-3025/

    Thanks Wally Greene..

I was reading this post before, it confused me. I dont know, if im going to find a civil surgeon to transfer my I-693 or just include my vaccination supplement to my AOS packet.

My vaccination record checked as "Vaccine History Incomplete" - "Applicant may be eligible for blanket waiver (s) because vaccination (s) not medically appropriate (as indicated Above)".. Almost everything is check in my vaccination record as "Not Age Appropriate" except for Influenza, check as "Not Fall (Flu) Season". MMR & Varicella as "completed" and TDAP as "Insufficient Time Interval".

Because of the incomplete check my husband and I just decided to find a civil surgeon for I-693 just to avoid RFE.. Do you think i really should do it?

    Thanks Again..

  10. On our case, we havent received any NOA2.. It's true that u have to pay $400 just for a copy of it. What we gonna do it to attach our NOA1, screenshot from USCIS website and letter from Embassy that I am eligible for interview..

We will also do an infopass to immigration office near us.

You can also read this to have more info on what to do.. http://www.visajourney.com/forums/topic/248800-missing-noa2-for-aos/

    Goodluck!
