Additional questionnaire form was not mandatory. Most of the question already covered by DS -230 form. I've heared if it is required for someone then they have asked to fill it up & submit during interview. From my point of view if they didnt ask then I will wait for their instruction. Best of luck too. I already waiting 1 month + ....

no, not yet. But they replied my mail that required additional processing is pending & they will contact with me as soon as the processing is finish. what about you ? When you had interview ? did they place you in AP ?
