You don't need the appointment letter/package to start your medical examination. All you need is the MNL# which I believe you have. I actually inquired from ST. Luke's and this was exactly what their response was "Normally, appointment letter is presented to us during the medical examination. However it is your discretion to have the medical examination without the appointment letter for as long as you know your case number and visa preference category." Just make sure you register online just like the guys said to make things easier. I have my fiance going 2mrw and she has not received her package either
