    Yup u r rite........but they didnt ask me to submit tht too.....untill i asked them..thts y i m saying tht....it might happen they will say tht to do so after 2 mnths...anyways wish u best luck.

    It was 24th october....co was female....if u didnt submit tht....email embassy asking whether u will have to submit tht or not.I m saying it cause in my case they didnt ask for tht.....then after few days later i emailed them asking if i should submit tht questionnaire form.....then they replied me to submit tht.....hope u get my point........best of luck & lets wait .....:)
