    Cross-posting from my profile page in case you don't see my reply to your question:

    I didn't have to ask them for an extra copy. When I went to pick up my results, they handed me three things; the sealed envelop, a CD, and an envelop that was not sealed which contained a copy of my vaccination records (Vaccination Worksheet DS-3025). Nurse said I'd need it for AOS.
