  1. Hi, I just submitted a photocopy of form 3025 the vaccination sheet given by Saint Luke's. It was said as long as you filed your AOS within a year of medical exam no need for the I- 693. And I rely to that. I do not have any problem at all. I cannot explain why others receive RFE for that. Or maybe it depends also what they see on the record. I have my medical last January 2013 then we filed AOS last May this year.

  2. Did you have your inteview at the embassy yet? You need your passport and a couple photo copies for CFO, so you either go before the interview at the embassy or after when you have youor visa.

    CFO:

    ALL exiting required to attend. There have been changes regarding CFO so for sure read through their website.

    Update: Please check the schedule of the counseling session before proceeding to CFO because it is country-specific and the slots are limited only to 15 per session (two sessions per day) for Manila and 12 per session (only one session a day) for Cebu. It is on a first-come-first-serve basis.

    Required documents & "OTHER DOCUMENTS" they ask for at CFO: copies of; CENOMAR, certified birth certificate, bio page of passport, visa page from passport, pictures together. From USC (copies of) - bio page of passport, birth certificate, divorce decree (if been married before) are the most common requests. The "other documents" clause can be ANYTHING with those nit-wits at CFO.

    ** From the Family Code of the Philippines: it's 21-25 years old for advice and 18-21 years old for consent. (Some are posted on VJ that they were not asked for this, yet others have been asked, so be prepared)

    What are the requirements needed?

    How much is the payment?

    It is fine to take seminar even the visa is not delivered yet?

    You are welcome JedNik and thank you too. I do bring a photocopied passport on the interview but they are not going to ask for it. They only need a valid passport then those required documents for interview. A photocopied passport was indeed asked for medical examination.

  5. In my case, I did not submit any photos at all during the interview. Photos that I submitted during the medical were the ones that the Filipino consul used and glued on my papers. Congratulations Jen!!! :dance: :dance: :dance:

    Thank You TatTot. Two of my photos were attached on the form. Then the photos i give on medical was on the the plastic pouch i saw they clipped on my forms.

  6. They didn't take any of the 2X2 photos from my wife either, she had them in a pouch with other pictures. Still has them.

    Interesting that they are making a point of having the photos pasted now.

    I was surprise also. I thought its only them that were going to attach it on the form. Only two photos that has pasted on the form they have from me today. I already submitted three photos on medical examination.

    I arrive USEM before 6 am today. I am on batch 3 already.

    Then fall in line and prepare your passport and the DS forms.

    PLease paste the photos on DS 156. i really thought they are the only one who attach our photos on form. But no worry in case you haven't pasted it yet.

    There were vendors there and you just pay 5 pesos for two pics pasted.

    What a good business!. Paste can be bought by 3 pesos only.

    But yes it is more than worth it. Then while pasting my photos i can really see they brought along their CD chest X ray. And i didn't brought mine since i was instructed to bring it only in the USA.

    Thank you that i read VJ K1 interview experiences here. That do not listen to all what you heard outside.

    After giving the passport and DS forms for the gurad to check we went inside again and another security check. Then give our forms and they attach number on it. I got 6077. And waiting begins.

    I first called to window 42 for finger scanning, 4 fingers left and right then the thumbs.

    Wait again then called on window 35 for Filipino interviewer. Give your NBI,CENOMAR and NSO birth certificate. Then the BPI receipt and the AFfidavit of support or form I-134. and do the interview.

    He asked about him, me and look for our photos. And asked why i only have few photos for two times he visit. I answered that's all i printed and the rest is on USB. He doesn's say anything and told me to just wait your number to be called.

    Then i am called on window 59, a young American maybe on 30's. Take my oath. He asked me about him, how did we met and how he propose. When the last time we saw each other. Then asked for photos. To be honest i cannot barely hear when he spoke so i need to ask his questions again.lol, he spoke far from microphone and look on monitor. I amthinking there were questions there? and his window is a bit "kept" unlike the others that is very open and we can easily hear the words. Then he say something again about domestic violence and told to wait to be called for further instruction.

    My name was called on window 72, numbers weren't flash anymore this time. Then the woman told me that i can return home and wait for my visa.

    And everything finishes around 10:20 am. Then before i left, i grab a t shirt with US embassy on it for 400, it is a good deal. A souveneir for me.

    For everyone, just familiar with the dates of his visits and the day he proposes to you.

    Hello there, do you mean the VACCINATION DOCUMENTATION SHEET? But for a copy of PDS given by the guard when we enter,no. Though in our VDS it has our PDS number. Then the nurse gave me a CD chest x-ray and the official receipt for medical fee. And returned the passport. Instructed to bring the VDS and CD chest x-ray result to USA then the O.R. during interview. That's how i remember but i will just bring them all on my interview this 29th.

  9. It took me two days also for medical examination but to total the hours it will sum up into a day. On my first day it took me 4 hours or less then on the next day ,maybe around two hours for the immunization shots and releasing. I got mine last January 14 & 15,2013. And i stayed in Lotus Garden Hotel.

  11. I did my first day of medical examination today. I went to Saint Luke's Medical Extension Clinic after taking my breakfast. I eat rice with pork tocino and hotdog. Then have coffee, water and juice. And oh i eat watermelon also since my fiancee told me i am ruining my system to what i eat. Then i arrive there by 8:43 am. I lined up there and prepare the required documents needed.

    PROCEDURES:

    ENTRANCE:

    1. Line up to get priority number.I got # 98. The guard will check your passport, photocopied biographic page of passport, appointment letter/ NVC instructional letter. Then the 2 print outs if registered online. Then gonna give applicant PDS for to fill up inside. Online applicants gonna have red colored number tag the others got blue.

    GROUND FLOOR:8:46 am

    2. Fill up the PDS and wait for number to be called.

    3. Once called, submit your PDS form and they will get one of your 2x2 visa photo. Then gonna finger scanning,digital picture taking and finger scanning. Then give you a sheet to bring to USA cashier for payment.

    FIFTH FLOOR:9:22 am

    4. Paid the amount of Php 9,366.00.Pedia applicants will pay 7,770.00. Then told me to got to vital signs. Others were told to go to chest X-ray and to other test.

    4a.US Counter 10:22 am, for eye test then the height and weight determination.

    THIRD FLOOR: 10:52 am

    5. Chest x-ray....remove all your jewelries and top clothing.. tie hair.

    FOURTH FLOOR: 11:11 am

    6. Blood extraction and Urine Test..

    FIFTH FLOOR: 11:28 am

    7. US Counter for Physical Examination. They were going to give you a another number this time. I got # 21, waiting at RM 10.. The doctor is going you medical history and such things. Gonna told you to get naked, look at your back then asked you to lay on bed. Just follow instructions..Just quicky!

    SECOND FLOOR:

    8.Immunization Interview and beign told what vaccines needed and how many.

    It all ended by 12:19 nn. Still gonna wait for my tomorrow's immunization but of course depending also to what my medical result gonna be.

    I just finish printing my DS forms 156,157 and 156K as well as the part one of DS230.

    I am wondering if there's a specific size for the paper?

    And if misses a question, is it okay to just write on it neatly?

    Then on DS-156 question #33: Do you intend to work in the U.S.? If i answered No on this,does it gonna affect any job hunting in the future if ever?

    then on question # 36: Has Anyone Ever filed an Immigrant Visa Petition on Your Behalf?

    Yes or No

    What is the correct answer for this? He already filed me a K1 last year but we withdraw it and then we refiled this year.

    How many original copies for NSO birth certificate, NBI and CENOMAR?

    And where will be it used for?

    Do she needs to wait for her packet from Manila Embassy?

    When is the best time to call NVC?

    When is the best time to call Manila Embassy?

    Is Transcript of Records (School) needed also?

    I will think again what other questions in my mind VJ family.

  16. The withdrawal letter we received has an A# on it.

    This is our question. How did we get an A#?

    What is an A#? where and when do we receive one?

    That letter that confirms petition withdrawal has an A# on top right of letter.

    Is this our A# and what does it mean?

    We are confuse of this A#.
