    how did you send the additional docs that the embassy were asking for? Can we send from US directly to the US embassy address? Hopefully you can help me. I have the docs ready but I'm afraid how and where to send it. The email that I received from embassy is not complete on how to send the docs, they're just asking for the documents.

    It is a good suggestion, but sound scary lol.!! Do you think they will not look at the authorization letter before signing ?or the authorization will just like a gate pass for someone authorize to ?

    I am a Naturalized US Citizen, petitioning my Mother and my minor child under IR5 and IR2. The Petition is in NVC stage, which i already paid for AOS fee of $120 and right now i am working on getting the documents done. However i have some concerns that hopefully someone can help base on your knowledge and experience. Here are some of my questions.

    1. SLEC (St. Luke ) medical requirements for Minor child is = they should be accompanied with parents (which i am not able to ) or by a guardian (my mom) with Authorization Letter or Power of attorney. My Questions are:

    a.) Is the authorization Letter signed by me and AMerican lawyer would be accepted?

    b.) or do i need to get this letter from the Embassy of the Philippines?

    2. SLEC Medical for my Senior Mom = can she bring someone with her inside to assist her with her medical ? what would be the requirements?

    3. During Interview : Can my mom bring someone inside the US embassy as her assistant and interpreter ? and what would be the requirements ?

  4. SSA is federal, has nothing to do with which state you are in... I would say you got an ignorant SSA person. We completed the SS-5 and turned it in with a copy of our marriage certificate and I-94, no issues, but then there shouldn't be.

    Maybe you are right that I got an ignorant SSA person. lol. But anyway, maybe in two weeks I will have my New SS card with my married name on it.

    I applied for my SSN after my marriage thinking that the SSA will put my married name on the card. However, they didn't.!.SSA still used my maiden name. They said that, they cant put my married name on the card yet since it should be the USCIS will change my name first.(eventhough i brought with me Marriage License). ( i do not know if every state has defferent issue on this case.) I am in North Carolina.

    I recieved my GC and went to the SSA to change my SS card name from maiden to married name.

  6. Will the name on the green card and EAD be the maiden or married name?

    If she gets a SSN in her married name will this be a problem with employers if it doesnt match the EAD name?

    Hello,

    Used the married name for Greencard and EAD and advcance parole application.

    When u are applying for an SSN before & after marriage,they will still used the maiden name. WHen u have your greencard then thats the time that you will change it to married name.

  7. she doesn't have a vaccine records, would this be a problem? she have all the shots from when she was a baby, and she graduated nursing...

    any advices. please... what should i tell her? and what should i tell her to do to have this vaccine records?

    If she cant find a copy of her vaccination before dont bother, it is okay. What we pay for the medical covers all the vaccination she needed.
