Hi, you only need ds 156, ds156k, and ds 157, no need for other forms. You can print it online or you can use a pen, if you forgot something, you can use a combination of both. No need to wait for the packet from USEM. As long as you have your manila case number, you can schedule for your medical and interview. I received my MNL# by August 2 and had the packet 21 days after it.
