  1. yes,, all u need is fill out the form and send it back to them,, and also, send a copy of ur proof of ssn..

The letter they sent me saying that I have passed the NCLEX-RN exam is the same form I'm gonna fill out and send back right to them via courier services? Please do correct me if I'm wrong. :help: The letter I've received goes like this:

    File #XXXXXX

    March 30, 2010

    MY NAME

    MY ADDRESS

    SUBJECT: Social Security Number (SSN)

    Dear MS. maiden name

This notification that you have passed the NCLEX-RN examination and that you have met all other licensing requirements except for submitting a valid US Social Security Number (SSN). Your SSN is needed in order to issue a permanent RN license.

Please submit your SSN on the form below. When you obtain your SSN, you are advised that you must also promptly report it to all appropriate agencies, such as California Franchise Tax Board.

If you do not provide your SSN to the Board of Registered Nursing within three years from the date of this letter, your application will be considered abandoned and your file will be destroyed.

    NAME: _________________________

CURRENT ADDRESS (if different from above): ___________________________

    US SSN:_____________________

I certify under penalty of perjury under the laws of the State of California that all statements, answers, and representations in this form are true and correct.

    SIGNATURE: ______________________ DATE:____________________________

When I fill out this letter/form, I'm gonna use my married name, current address (in CA) and signature (of course, also in married name). Then, can I include in the packet the change of name and address form (hard copy/printed and filled out) together with the proof of US SSN and a photocopy of my marriage certificate? So that they're all in one package? Can that be enough? Thanks in advance! :)

  2. Apparently after 12 hours it is still not in the system. I may call in the morning. My Fiancée's passport number is just seven digits without an alpha code (Unless you count the "XX") and I do not know how they are handling this.

I know I am a little impatient as I want this done NOW, but I also accept that there will be unforeseen complications and I am trying to head off as many as I can.

A classic interaction between Western rush mentality and Philippine "Mamaya na."

    What an adventure.

include the XX on the passport number when trying to schedule

  5. just bring the letter they sent you saying that you passed nclex-rn. once you have ur ssn, fill out the form, submit proof of ur SSN and in few days u'll receive ur license.. :) they also have the change of name and address form which you will have to send together with ur marriage certificate as proof of name change. . i believe u can submit these through fax...

Thanks for the advise! I have no plans of endorsing my license to another state. :P My hubby lives in California :) I just got confused on sofaraway's case regarding retaking the NCLEX-RN exam. I wanna prepare everything before migrating in CA and eventually work as a RN. I guess I have nothing to worry about anymore. All I have to do is bring the NCLEX-RN exam result's letter, present a US SSN once it's available, obtain my permanent license and work! NO more red ribbon and stuffs!

  6. My husband just submitted copies of his Birth Certificate and Divorce Decree upon sending the petition to USCIS. He sent me scanned copies of it and I brought it to my interview but the CO didn't ask for it.

same case with you. had my interview yesterday and they didn't even ask for my fiance's birth certificate. Divorce decree was submitted to the USCIS and US embassy has our files. pre-screener said that they had the divorce decree on file and never asked for another copy (although i prepared a scan copy just in case they ask for it) .. :)

  7. Had my medical on SLEC last June 14&15 and they returned my passport.. they no longer keep passports..

I read recently (couple weeks or so ago) that SLEC no longer keeps and forwards passport to USEM. They stated SLEC imformed them it was a new rule for them to return the passport to the person so it can be used as an identification to enter USEM for interview.

Maybe someone with recent experience at SLEC can chime in on this.

  8. we will be reaching 5months since our NOA1 tomorrow and yet there is no NOA2. when my fiance called the lawyer who is representing our case, the lawyer said we still have to wait 2 more weeks after the 5month period. only after that 5month and 2 weeks without NOA2 that our lawyer will call USCIS. is it how it is done?.. read posts here from VJ that some are calling just few days after they reached the 5th month mark. can we call USCIS without our lawyer's knowledge? felt like we cant do anything because of him.. thanks....
