Guys, from Philippines those done with interview, is the I-134 needs to be notarized? How about the W-2 and ITR 1040 Form it should be provided both of one will be enough but substantiated with Employment Letter and bank statements... your reply is great appreciated. Thank you.

Chona, Here is the comment from the I-134 Form Instructions:
"You must sign Form I-134 in your full name. (Note: Signing Form I-134 is under penalty of perjury under U.S. law). For this reason, it is not necessary to sign Form I-134 before a notary, nor to have your signature notarize after you sign it."
Your Fiance should send you the original signed form.
