    oook I just opened it and I have the chest x-ray, the ds3025 (vaccination documentation worksheet) and stapled ds 2054, ds 3030, ds 3026, ds 3025 and an invoice. I bet that was the document you were missing in the yellow envelope that he had to take from your white envelope. Hopefully they got a copy of ours lol.....or i have it here!
