I-130 / I-485 / I-864 AOS package checklist

Hello,

I am in process of filing Adjustment of Status for my mother who is currently in U.S.A. After doing some research this is what I have for supporting documents.

 

Please note this checklist is specific to my situation. I would appreciate if someone can let me know if I am missing anything.

 

Please advise. Thanks!

 

I-130 Checklist

  • Signed I-130 form
  • Petitioner's birth certificate showing my both father and mother's name.
  • Copy of petitioner's naturalization certificate. 
  • Copy of father's death certificate.
  • I-94 information.
  • Form G-1145 (E-Notification of Application/Petition Acceptance)
  • Filing fee for Form I-130 ($535)

 

Questions I have on I-130

  • Do I need to send my and  mother's photos with I-130?
  • Copy of mother's birth certificate is it needed here?

 

 

I-485 Checklist

 

  • Signed I-485 form
  • Beneficiary's 2 photos with name on the back
  • Photocopy of all pages of beneficiary's passport.
  • Copy of beneficiary's birth certificate
  • I-94 information
  • Form I-693 medical exam report (Sealed envelope)
  • Form G-1145 (E-Notification of Application/Petition Acceptance)
  • Copy of father's death certificate.
  • Total Filing fee of $1225 ( Filing fee -$1140  + Biometrics fee -$85 )

 

Questions I have on I-485

  • Do I need to send copy of my mother's old passport containing her expired USA non-immigrant visa?

 

I-864 (Affidavit of Support) Checklist

 

  • Signed I-864 form
  • Copy of 1040 form for year 2018
  • Copy of 2018 - W2
  • Copy of 1040 form for year 2017, 2016 (Optional)
  • Copy of W2 for year 2017, 2016 (Optional)
  • Copy of last 6 months pay stubs
  • Copy of last 6 months bank statements
  • Employment verification letter.

 

Questions I have on I-864

  • Do I need to send copy of my birth certificate?
  • Do I need to send copy of my naturalization certificate?
  • Do I need to send copy of my passport pages?
