DS-3025 Complete Vaccination Record (as per the instruction to Form I-485, K1 Visa holder does not need to submit the entire medical report if the medical examination was conducted within the past year. As my medical exam was completed on April 7^th 2017, I only included the vaccination supplement)

11. I-797 Copy of Approval Notice for I-129F

12. I-864 Affidavit of Support including the most recent tax returns and Form W2’s, IRS transcript, paystamps, supplement with additional information, copy of Sponsor’s Certificate of Citizenship
