questions on EAD/AP renewal - doing it myself

Hi Gurus, i am about 85 days about to expire employment based EAD/AP combo card (485 filed after jul 2007). I would like to have myself apply for renewal for both my application and spouse. I have couple of questions and any help in this regard is appreciated:

1. if same copy needed twice for AP and EAD; then 2 copies of say DL/EAD/Form G-1145.. has to sent in different sub-envelopes as if they are totally two separate applications?

2. which one is better e-file vs traditional mail?

3. do we need to submit any proof that we paid 1040$ fee?

4. i looked at USCIS website and other forum posts and figured it is ©(9) for the #16 on i-765 but had a question on which address to send (look at USCIS website; but confused me a little due to no separator in the table they had for this..)

i live in california so it would Phoenix, AZ for EAD. I am still trying to figure out the 797 # to find out address to send AP.
