Missing documents at interview (medical supplement)

Hello All,

I need some help and hopefully someone has experienced something similar to my current situation.

Me and my wife were interviewed Jan. 13 at the Downtown LA office. Everything went great till the interviewer asked for my wifes supplemental vaccine record which we havent had transcribed at the time. He gave us a letter which had a checklist and circled the vaccine as missing. So three days later we send it back to the address provided on the form, problem is, we forgot to include the checklist that was provided! Dont know how that would affect the process but we sent the letter seperately couple weeks later just to be sure. Its been almost 2 months and we havent heard anything back.

Anyone else experience this?
