Hello,

I have two questions related to AOS filing.

I read from instructions about form G-325A Biographical information that we should include one original signed form and one copy of the signed form.

We have lost the original G-325 forms that we sent to California Service Center when we started the K1-visa process. Does this mean that we should include originals that we sent for K1-visa? Or that we should send one original new form as well as a copy of it? But they say one should not send any originals....

Also, when I had my K1-medical exam I received Vaccination Document Worksheet (for use with DS-2053). Do I attach the copy of it with the AOS application or should I contact a surgeon and have him write it to different form?
