· Copy of federal income tax return, including W-2's for the years 2011, 2012, 2013

· Employment Certificate

· Copy of most recent pay stubs

· Copy of U.S. spouse passport page

I-765 Application for Employment Document

· Form I-765 Application for Employment Authorization

· Copy of Passport with nonimmigrant visa

· Copy of Electronic Form I-94

· Two (2) Passport size photos

· Certified copy of Marriage Certificate (to note the name change)

I-131 Application for Travel Document

· Form I-131 Application for Travel Document

· Copy of Passport with nonimmigrant visa

· Copy of Form I-797 Notice of Action (Approval Notice)

· Copy of Electronic Form I-94

· Two (2) Passport size photos

· Certified copy of Marriage Certificate (to note the name change)

Copies of documents submitted are exact photocopies of unaltered documents and I understand that I may be required to submit original documents to an Immigration or Consular officer at a later date.

Is your foreign birth certificate in english? If not, need to accompany that with translation.

You only need to send the one copy with the I-485.

The OP added in a few extras that are not required, such as proof of citizenship for the USC petitioner/sponsor with the I-864, the copies of marriage certificate with the I-765 and I-131, and approval notice from something (I-129F?)with the I-131. None of those are needed.

Hi, I was not giving the i 94 form to fill at POE. What do I do about that while submitting for AOS?

sorry I don't have the link but search I-94 here and you will be directed to the site where you download and copy the electronic I-94.

nothing to fill in, just get at least 3 copies, you need one each for I-485, EAD and I-131.

Thank you for posting your cover letter! This is exactly what I need right now, have just compiled the forms (still have final checks and bits to do for the I-864) but was wondering how to set it all out. Thank you very much for sharing
