We applied for a CR1 visa. I am Italian and she is American. We got married last september in Miami and then moved to Italy.
We started the process as per our timeline.

Where to sleep:
I went to areamare.org. It's a bed and breakfast very close to the consulate. Not more than 5 min walking. It's in a bulding on the 3rd floor. The room are big but kind of old. There is a big restroom with shower, sink, bide and a window in the bathroom.
The place was overall comfortable and neat. DO NOT EXPECT a 5stars hotel.
I paid 55euros per night.
To get there form the airport i took a 3euro ticket for the alibus to piazza Garibaldi then the metroplitana till Mergellina (4 stops). The B&B is 10 min walk from Mergellina station.

Medical examination:
I had the appointment at the Istituto Varelli at 8:45am. I was there at 7:30am using a cab from areamare. Taxi ride was 10 euros. Get there earlier does not give you any advantage.... read complete review

It was easy, we have to say. The day before you do your medical, we got there at 8am out before 11am. They told us if anything was wrong they would call before 2pm if not no phone call. The next morning our apt was at 9am but we arrived at 830am. It was a 3 step process. After you enter the embassy they direct you to the appropriate area. There we went to the first person, they ask a couple of questions, get documents, then direct you to a second person, where they take all the other necessary documents and do fingerprints, pay fee if necessary. Then the interview. for us 3 questions, how we met, when was the wedding and what will we do after in the USA. Approved pick up everything at mailbox office at 5pm...we were done in under 2 hours

Daniele had a very good experience in Naples. He had his medical exam on Monday and the interview on Tuesday. He highly recommends that you take a taxi to the hospital for the medical exam. He also recommends the B&B La Dimora di Nettuno (http://www.hotelpernapoli.it/). It is very close to the consulate.

After the medical examination, Daniele and the other 5 people were told to go to the consulate the next day at 8 am because they were so few people. Daniele handed over his passport to someone who checked that all of his papers were in order. At about 9:30 he was called to the interview window and asked to swear that everything in his application was true. That was it - the end. The consulate could not guarantee that the visas would be done that afternoon so Daniele opted to have his visa delivered via express mail at the cost of 40 euros. He received his passport with visa and packet on Friday.

Medical: easy but long. make sure you get all possible shots before you go (MMR and tetanus) because its super expensive to get even just one there (I had all vaccines, except for one of the MMR - which is a 3-vaccine shot.. they charged me like 50 euros extra). Other than that, the doctors are really nice.
Interview: I had 2 J-1 visas before, and one of them had the 2 year rule on it. The advisory opinion said i wasnt subject, but the consular officer wasnt satisfied because the visa said otherwise. Fortunately, I also had a no objection statement from the Italian Embassy in DC saying they didnt care whether I had the 2-year rule on it or not - when I remembered I had brought that letter, everything went smoothly and the consular officer only asked a couple of quick questions and was really really nice and understanding.

I got my medical the day before the interview. I got at the Instuto Varelli at 7.40 am and I was out by 11.30.
The doctores are nice and friendly. I got blood test, X Ray and they gave me the 2 shots I was missing and just a general control at the end. No particular questions.
Even at the Consulate everything moved pretty quickly. My appointment was at 10 am and I showed up at 9.30...By 10.20 I was out. They didn't ask for any eveidence, they already had the whole package from CSC. The Consular was very nice!! He just asked me if my fiancee was still deployed and where, when he was coming back and when and where we were planing to get married...Then he just told me "Well this time you won't have problem to get to the States cuz you will have your Fiancee Visa" (last time I tryed to enter they deny me entry) and he made me a big smile and showed me two thumbs up...Haha! He was a pretty funny guy! Anyways I got say everything went very good!!
