Part 5 says "List all your children" that includes the ones born in the USA, you note in their immigrations status "Born in USA"

Does this portion ask for the applicant's children who are also applying for the lifting of condition with him/her?
I just had a kid (who's automatically a US Citizen) with my petitioner-husband and was wondering whether i need to put my child's name on here.

I did. I had no problem at all. Seattle was my DO.
I held on to it while waiting for my aos interview coz i didn't wanna risk going to the interview without it. It takes a couple of weeks for the Phil consulate to process it and send it back.

You don't need to get any vaccination before your medical. During your medical, St. Luke's will give you all the vaccinations you need depending on what's available. Those vaccines are included in the fee that you're going to pay them.

I used my current last name (hubby's last name) and put my maiden name as "middlename" which is what we normally do in the Phil.
My name on my GC then matches with that of my passport's.

July 31 - "Welcome to America letter was mailed" email
August 8 - "This case has been approved, an approval notice was mailed" email
August 17 - GC & Welcome letter arrived together (of course bearing our old address
) despite filing all the necessary forms, calling the 1800 # and receiving a letter confirming the addy has been updated)
Oh well
glad it's over for now.

I've got 2 questions for you:
a. How did you know your work permit was returned to USCIS? Did you call 1-800# or thru online status?
b. What USCIS address did you use to send the letter to?

On July 31 I received an email saying a welcome letter was mailed and on August 8 I got another email saying the approval notice was likewise sent.
It is now the 9th and I have'nt received the welcome letter yet. We've changed our address in April, sent the forms (AR-11, I-865) and called the 1-800 #. I got the letter confirming that our addy was changed in the system. However, I am still thinking they sent it to our old addy.
While I am willing to wait for a few days more, I'd like to know in advance what needs to be done in case an infopass appointment next week confirms that the Welcome letter/GC/Approval Notice is returned to USCIS.
Thank you in advance for any advice you can give me.

hours.
Had interview June 23, 2006 and was approved. Three weeks later no welcome letter no greencard in the mail. Had infopass July 14 and was told my case is pending FBI namecheck. I thought I'd have to wait forever but glad that it's over.

i got recently my interim EAD for 90 days. After 90 days can i resiisue another one or this is the only one given??

I was able to get three 90-day interim EADs. I was told i could only get three. Was glad my one-year EAD was finally approved before my 3rd interim EAD expired.

I was interviewed June 23. The Officer said he's approving my case, took my I-94 and my EAD card but didn't stamp my passpot (didn't ask for it coz I know,based on others' experiences, SEattle DO does not stamp passports anymore). He said i would hear from them in a week. Three weeks later, NOTHING. So went to an infopass and was told my case is pending FBI name check !!!!!! Why the officer didnt tell me about it i have no idea. I waited for almost a year to have this interview and after letting out a sigh of relief knowing i was approved, here I am in this never-ending waiting game again.
Told the officer my passport was not stamped, he said the interviewer should have stamped it since he took my EAD. Anyway he probably felt sorry for me and reissued an EAD good for the remaining validity period of my EAD card.
I suggest you just wait 3 weeks and go for an infopass to find out what's going on.

Meanwhile, I see that Atlanta processing time for AOS petitions has jumped from August 2004 to April 2005 in 1 month... Thanks to applications like mine, that were transferred to California to "speed up" the process.
This is so ridiculous...

Yeah.. its ridiculous... Seattle DO is now processing those submitted July 2005 and am here stuck at CSC without even my original EAD application acted upon. It's been EIGHT MONTHS already!!!!

I know how you feel. In the thread of EAD approvals, i too, was one of the few left unapproved and now i am being left out again. *sigh*. I wish they didn't transfer mine to CSC and just called me for an interview in Seattle. Those who had interviews there had an average only of about 4-5 months waiting time. Now, my plan to visit Phils soon will not push through.
anyway, hope you get a favorable response from CSC.
