Personally, I would just include the ones you have with both names as long as you have plenty of other strong evidence. USCIS doesn't expect every type of evidence to go back to the date of marriage.
In our case, I only included water and phone bills because those were the only ones that showed both names, and none of the other bills are in my husband's name (the immigrant).

The NOA is the extension letter. The NOA will have a typed paragraph or two on it stating the your GC has an extended validity of 24 months. Again the NOA and extension letter are one in the same. Be sure to make several copies and put away the original for safe keeping.

Yes. You are required to bring that letter along with the green card but it works the same as the card did before it expired -- you can travel.

You don't need to stamp your passport. This is only needed when your extension letter expires.

Your passport doesn't get stamped after receiving the NOA. When your Green Card expires, you just carry the card and the NOA extension letter with you in situations where you need to present your card until your case is approved and you are sent the 10 year card.

N/A is always going to be the safest bet if you are concerned that the person reviewing your documents might think you forgot to fill in a box.
And yes, you leave everything above 'START HERE" blank.

I don't know if you've found your answer yet but it says within this link https://travel.state.gov/content/dam/visas/iv-dv-supplemental/Havana-GEO.pdf that you can pay your fee on the day of your interview.
If you've heard something different from other people, would you mind sharing? I'm not a Cuban, but I also have to go to Georgetown, Guyana for my medical and interview because these things aren't done in Suriname apparently. (I sincerely doubt it'd be different from what is stated within the instructions themselves)

@Greenbaum I emailed the embassy for the P3 and received it today. It's the exact same one as the one you sent me previously- except with the obvious differences such as my name and the date. Apparently, they'll allegedly choose the interview date for me (at least, that's how I understood it).
How exciting, onto the next step we go~!

Lots of people know when the next shipment is. It's next week on Tues AND Wednesday. We think the cut off is the Wednesday before the shipping date so you may have just made it. Go to the first page, first post and toward the bottom of that post will have information about the shipping.

With case number you're able to check your visa status at CEAC and see when it left for the embassy, when the embassy got it, when it's ready for the next steps and later on when the visa is issued. NVC doesn't do checks nor it approves or deny K1 visas, they just put you in the sistem and forward your case to the respective embassy to be processed further, the interview stage. Depending on the country you're able to book your medical with the case number and also get your interview date.

Now just hold on there a minute, Shiran! Who in the heck do you think you are using logic and reasoning? It is YOUR right as a citizen to get NO information, be told NOTHING of significance when asking legitimate questions about your case and being needlessly delayed re: your petition. I think you owe the uscis a sincere apology.
As for Tier 1 CSRs? About the only value they MIGHT offer is providing the local time (and maybe not even that).

You can proceed with your medical and scheduling your interview. But call uscis to get your NOA 2. You need that on your AOS.

As an ER nurse and PHRN for 12 years this does not surprise me at all. The only thing does is how cheap your Zofran and Propofol were!
This is also charges billed to insurance. If you had no insurance at all and were billed direct pay it would prob be 10-15% of what it is.
The insurance issues raise the price of big pharma too. They will bill Blue Cross for $946 for a med that out of pocket cost is $33.
And I hope your wife is feeling better!
