Put down BOTH his last names as last names. Do not put one as middle name --- make it so that it appears exactly as in his passport. I changed my last name after marriage, but my visa and permanent resident card still came out to my maiden names, First, Middle + 2 Last names (one of which is a compound one, looooooong).
