    You can apply any time now if you've been a permanent resident for 3 years and still married, living together in bona fide marriage with your spouse.
     
    If you file N-400 it now, you'll have to provide bona fide marriage evidence. If you file 90 days before your 5 year anniversary as permanent resident (conditional card issue date), you will have to provide fewer documents.

    Depends on how you booked your flight. 
     
    If you booked it all on a single itinerary: Houston-Los Angeles-Manila, then time your test using your departure date/time from Houston.
     
    Be more careful if you booked your domestic flight and international flight separately. Someone I know made this mistake: booked a domestic flight Memphis-Atlanta, stayed overnight in Atlanta, and booked a separate flight Atlanta-Incheon-Manila. He thought the test he took in Memphis would meet the requirements but it was rejected by the airline because he took his test too early. He ended up needing another test and rebooking his international flight.
     
    In any case, the 15 minute antigen test solves these timing issues. Go to Houston airport a bit early and get the antigen test done a few hours before your flight. I'm sure you can find a testing center inside or near Houston Airport that can quickly issue official-looking test results valid for international travel.

    If you can submit any documentation to show that your finances co-mingle, that's always good. I submitted our joint checking account statements and I personally chose to black out the account number (I did the same with other stuff that had account number). 
     
    As for pictures, you don't need to submit a whole lot. Send maybe 10 from different times and events during your marriage. Make it a mix of couple shots and other photos taken with family and friends.

    My wife, a Filipina, did the same thing.  She tried to use her previous last name as her "new" middle name.  She got through Biometrics, but, at the AOS interview, it was changed back to her original middle name.  The Officer noted that only the last name can change at this point.  When she files for citizenship, she can change it to whatever she wants.  This occurred yesterday btw.
