    It can be herself, or u or ur attorney, in tis case coz u already use attorney, she need to fill the form n choose ur attorney as her agent. Then aft she send tat form there will be more to do, such as pay the fee for u n ur wife then more paperwork. Well tats it :) u can ask me more if u need anything
